CHCANYS’ mission is to ensure that all New Yorkers, including those who are medically underserved have access to quality community-based health care services including a primary care home. Members are migrant and community healthcare centers throughout New York state. Membership: $500 (Associate-Not-for-Profit Organization); $2,000 (Associate-For-Profit Organization); $10,000 (PHSP Health Plan Associate); $75 (Individual).
